Leora accepts insurance from all major commercial insurance providers, and Medicaid. What treatment costs you depends on your plan, so contact your insurance provider to find out what it covers.

Checking your coverage

Your insurance provider can tell you whether your plan covers addiction and mental health treatment, and what type of coverage it has. This website never asks for your insurance details.

Ask your insurance provider which levels of care your plan covers, what your share is likely to be, and whether prior authorization is needed before you start.

Admissions can also check your coverage while you are on the phone. Have your insurance card with you when you call.

Bring to your assessment

Photo ID, your insurance card, a current list of your medications, and your discharge plan if you are coming from another program.

Which plans do you accept?
All major commercial insurance plans, and Medicaid. Coverage differs from plan to plan, so contact your insurance provider to find out what yours covers.
What will it cost me?
That depends on your plan, your deductible and how much of it you have met. Leora does not publish prices. Your insurance provider can tell you what your plan covers, and admissions can take your questions when you call.
What if I do not have insurance?
Say so when you call. Leora accepts Medicaid, and Ohio expanded Medicaid to cover more people on low incomes, so check whether you qualify if you have never applied.
Why do you not list the carriers?
Leora accepts insurance from all major commercial insurance providers. Plans from the same carrier cover different things, so the answer for your plan comes from your insurance provider.
